Registration Information The registration fees include attendance at all sessions, meeting materials, continental breakfast, box lunch, and refreshment breaks. Registration may be completed by mail, fax or via the internet. The early registration deadline is May 23, 2010. A confirmation letter will be sent upon receipt of the completed registration from within 7-10 business days. Payment can be made by check, MasterCard, Visa, or American Express. Payment must be received prior to admittance to the meeting. Online registration will be available beginning in early February. Registration Fees:
Cancellation Policy: If cancellation of your registration is necessary, you may send written notice to the registrar or call (801) 525-0231. Cancellations made by May 23, 2010 will be subject to a $35 administrative cancellation fee. Substitutions, however, may be made at any time. When calling, be sure to ask for and retain your cancellation number. Confirmed Registrants who do not attend the meeting or who cancel after May 23rd are responsible for the entire registration fee.
